i would go with 4, 8, and 12. i have experienced wearing my hair up and it makes you look bald. if you wear pig tails you will look little. hats make your hair look bad so i wouldnt wear one. as for the clothes a dress would be too dressed up but you dont want to be too under-dressed either so i would go with a pink shirt and jeans.
